It’s That Time Again! Chapter Achievement Awards and Master Questionnaire

CHAPTER ACHIEVEMENT AWARD

California’s goal for Chapter Achievement is 100% electronic reporting and Level 1 for all chapters! The newly revised Chapter Achievement Award 2012S form reflects the change of the National Society’s fiscal year to the calendar year. To align to the new calendar year, this year’s 2012S CAA form is only to include activities and financial contributions occurring between March 1, 2012 and December 31, 2012.

Do NOT include any activities for January or February.

The new 2012S form is now available to complete on e-Membership. Using the electronic e-CAA form on e-Membership you can complete the form now and go back and edit as many times as you need until the deadline. Please remember to click on “Save & Tally” each time before exiting the e-CAA, to save information, changes and to tally your points. A copy can be printed for your records. Don’t wait until the last minute… check it out now!

Section 1 is worth 40 points! Make sure you review the rules regarding resigning unpaid members far ahead of the December deadline to give a delinquent member the required 30 day grace period.

Don’t forget to process deaths, transfers and drops before December 1. Please review the NSDAR National Handbook and Bylaws available on the Forms and Publications link on the NSDAR Members’ website for more information.

Be sure all Section 3 financial contributions are received by the state treasurer before the December 31, 2012 deadline, better yet … write one check and include all Section 3 contributions with your dues on the State Remittance Form due December 1, 2012.

In order to earn chapter achievement award points in Section 5 for the 2012S year, chapter and individual member optional financial contributions must be submitted to the Development Office using form DEV-1003. The form(s) and check(s) must be received by the Development Office before December 31, 2012. Allow sufficient mailing time.

Preferred – Option 1:
The “E-Membership Electronic Method”
The deadline for the e-CAA 2012S thru e-Membership is 9:00 pm on January 31, 2013, but you are encouraged to complete the form earlier in January.

Option 2:
The “Paper Method”
The signed copies of the 2012S CAA form, available on the NSDAR Members only website, must be received by the state chairman by January 30, 2013. Allow time for mailing. This deadline is firm.

MASTER QUESTIONNAIRE

The 2012S electronic Master Questionnaire (eMQ) is online and ready for you! The reporting period for chapter activities and donations is from March 1, 2012 to December 31, 2012. However, the Official Membership Count is determined on a calendar year and it is taken as of January 1 each year. When using this electronic format, the Official Membership count will automatically be added to each page of the final report when it is downloaded. The deadline for MQ completion is 9 pm on January 31, 2013, but why wait… finish early!

Log in to e-Membership, or go to http://members.dar.org/mq and log on, click on Launch Master Questionnaire, then click on Instructions, after you’ve read this section… you’re ready to Open Master Questionnaire. The eMQ can be opened and re-opened as many times as you need to complete. When you have your final version of the eMQ completed, click on the Mark Survey Complete link. You can obtain a copy of your chapter’s report by using the Export feature in the eMQ.

Bee Sure to Take credit!
Many programs and activities can be reported to more than one committee. An example is a program given by a docent from DAR Museum on Historic Quilts of the DAR Museum and the local newspaper covers the meeting and prints an article. This program can be reported to the DAR Museum committee page, the American Heritage committee page and the Public Relations and Media committee page.

Please provide detailed information for those committees that request summary answers in the space provided on the committee page. The assumed deadline, unless established by the committee, for contest or supplemental reports is February 1, check with specific committees.
