The Chapter Conference Credentials Form is now available on the CSSDAR Members’ website under Forms and Publications, State Forms.
Complete the form immediately upon election of your chapter’s delegates and alternates and mail to the address on the form by the deadline. Remember, do not elect a member who is a state officer, honorary state regent, district director, or chapter regent, as they are voting members by virtue of their offices.
Be sure to read the instructions on the form carefully regarding substitution for a non-attending chapter regent and changes to delegates after a chapter election.
This form must be postmarked on or before FEBRUARY 1 with NO EXCEPTIONS. Chapter delegates’ and alternates’ eligibility are subject to verification by the credentials chairman. Eligibility includes the chapter’s on time payment of National and State dues, Chapter Conference Fee, and minimum one year membership in the chapter. Contact the state treasurer regarding dues and fee payments, and the state registrar for membership questions.
