Dear Chapter Regents:

It is time to submit your Annual Chapter Officer Report Form to the State Corresponding Secretary.  The information will be used to create the 2013-2014 State Yearbook and Directory.

This year, the process has been made easier for you. Each chapter will only need to complete one form.  This means that you will ONLY complete the State form. No need to complete the NSDAR form, because this officer will forward your changes to NSDAR for you!

1. The Annual Chapter Officer Report Form can be found on the CSSDAR Members’ website under under “Documents and Forms/State Corresponding Secretary.”

2.  The form can also be found under the “Chapter” drop-down menu in the Secretary’s Toolbox.

3. Completely fill in the form and email it to corresponding@californiadar.org, OR send it via regular mail to the State Corresponding Secretary:

3. Do not send pictures. The  webmaster previously sent information to regents regarding placing officer photos on chapter websites. If you have questions about this, please contact webmaster@californiadar.org.

4. Remember, do not send your chapter officer information to the National Society.  The State Corresponding Secretary will do that for you.

6. The deadline to send your completed form to the State Corresponding Secretary is June 1, 2013.
