Dear District Directors:

It is time to submit your Annual District Officer Report Form to the State Corresponding Secretary. The information you send will be used to create the 2013-2014 State Yearbook and Directory.

1. The Annual District Officer Report Form can be found on the CSSDAR  Members’ website under “Documents and Forms/State Corresponding Secretary.”

2. Please completely fill out the form and email it to corresponding@californiadar.org,  OR send it via US mail to:

3. Do not send pictures.  Photos will not be included in the 2013 yearbook.

4. Deadline to send completed forms to the State Corresponding Secretary: June 1, 2013.
